I haven't tried it myself, but I suppose the image url has to be between the labels.

Something like this:

phrase [img]url[/img]

I have the same trouble (I've been trying to change the pic in my sig for some time now). I PMed hobbes about it, but he hasn't responded in a month. I guess we're just stuck with things how they are now.
